No Further Action doesn't always mean the end of the story
A decision to take No Further Action (NFA) doesn't mean someone has been cleared. It means there wasn't enough evidence at that time for the case to continue.
If new information, evidence or intelligence comes to light, EQUISS may identify further lines of enquiry and, where appropriate, share relevant information with the police or other agencies.
Just because a case is closed doesn't always mean the questions have been answered.
What is an Independent Case Review?
EQUISS may undertake an Independent Case Review where there is a clear need for external scrutiny.
Our reviews provide an objective assessment of what has happened, how concerns have been handled and whether further action should be considered. They are designed to bring clarity, identify gaps and help ensure that important issues have not been overlooked.
An Independent Case Review may include:
- Reviewing evidence, records and documentation
- Identifying gaps, inconsistencies or missed opportunities
- Gathering additional information where appropriate
- Assessing how an organisation, employer or governing body responded to concerns
- Making recommendations to improve safeguarding, accountability and future practice
- Helping individuals understand reporting, escalation or other available options
Our reviews are led by experienced safeguarding specialists and former senior investigators with extensive experience of complex investigations, serious incidents and independent case review.
When does an Independent Case Review happen?
Independent Case Reviews are reserved for cases where external scrutiny is likely to add value. Every referral is assessed individually, taking into account the nature of the concerns, potential risk, available evidence, safeguarding considerations and whether an independent review is proportionate and appropriate.
Not every concern will require this level of intervention, but everyone who contacts EQUISS will be listened to and supported.
Our confidential support line remains the first point of contact for anyone seeking advice, guidance or advocacy. Where an Independent Case Review is not appropriate, we will help you understand your options and, wherever possible, provide practical guidance or signpost you to the most appropriate service.
Independent Reviews for Organisations
EQUISS also undertakes Independent Case Reviews on behalf of governing bodies, equestrian organisations, employers and businesses seeking an external, impartial assessment of safeguarding concerns, serious incidents or organisational decision-making.
Independent scrutiny can strengthen trust, improve transparency and provide confidence that concerns have been handled fairly, particularly where conflicts of interest or perceived bias may exist.

Independent Case Reviews do not replace police investigations, legal proceedings or emergency services, and EQUISS cannot determine legal outcomes.
However, independent scrutiny can play a vital role in strengthening accountability, highlighting learning and helping ensure that serious concerns are examined thoroughly and without bias.
